LPG Cylinder Gas Filling Plant Fire - Asia

Overview

An explosion occurred during routine operations at an Asia-based Liquefied Petroleum Gas (LPG) cylinder filling plant, resulting in a catastrophic fire that destroyed the entire facility. Tragically, the incident led to one fatality and injuries to two other individuals.

Envista’s Role

Envista was engaged by insurers to determine the root cause of the explosion and fire. Our scope of work was later expanded to include evaluating regulatory compliance and supporting the insurer’s legal team with expert analysis and reporting.

Investigation & Findings

Envista's forensic engineers determined that the incident originated at the large cylinder filling line. During operations, a cylinder fell and struck a small-bore gas pipeline connected to an LPG supply rim. The impact dislodged the pipeline, causing a significant release of LPG into the atmosphere. An ignition occurred almost immediately, resulting in a fast-moving fire that engulfed the filling shed and quickly spread throughout the site.
